How do I activate the Gold plan that is complementary with the Platinum plan?

When a Platinum member completes registration for DeleteMe membership, within their member dashboard they will see two datasheets. One will be the Platinum datasheet with their name on it, and below that will be a Gold datasheet they can use for a spouse or family member.

The main member will have to complete that datasheet out on behalf of the spouse/family member.
